Telecare offers several different types of programs within several communities throughout Washington, Oregon, California, Arizona and Nebraska. These include Community Based Programs, Acute Programs, Crisis Programs, Subacute Programs and Residential Programs. I have been doing my work in a community-based setting, this program is called Telecare ACT of Oregon or TACT.
Assertive Community Treatment (ACT)
The ACT model is a well-established approach for providing community-based services to individuals with complex needs. Services include assessment, services planning, housing assistance, pre-employment and vocational services, psychiatry and medication management, co-occurring disorder services, peer services, and assistance building natural supports.

What does "Community-Based" mean?
Community Based treatment is facilitated in the community, all of Multnomah County is where the Oregon ACT team services members. From Gresham to North Portland, team members take on a daily caseload and then meet the member where they are at. This could be their home, a shelter, a hospital, psychiatric facility, care facility, treatment facility, at a homeless encampment, a family members, hotel or coffee shop. The ACT team supports members with ADL's, skill building, appointments, legal issues, housing, navigating various systems. Harm reduction and maintains a safe and stable environment where recovery and quality of life can thrive. |
As a Vocational Specialist for Telecare, I also work coincidingly with OSECE and follow the 8 IPS Principles within my work. The IPS Model requires that I do 95% of my work focusing on employment and education.
Vocational Specialists follow the IPS (individual Placement and Support) Model. The IPS model has 8 course values that they work by:
- Competitive employment - Jobs anyone can apply for and offers minimum wage.
- Systematic Job Development - Employment Specialist visit employers based on the members preferences.
- Rapid Job Search - After an assessment, the first face to face contact with an employer is within the first 30 days.
- Integrated Services - Members are integrated into the treatment team.
- Benefits Planning - Employment Specialist help members obtain personalized information about working with SSi.
- Zero Exclusion - Members are not excluded based on readiness, diagnosis, substance abuse or psychiatric hospitalizations.
- Time-unlimited Support - Job plans are individualized, and support continues for as long as the member wants or needs support.
- Work preferences - All jobs are based on the members preferences, not on the Supported employments preferences.

Reflection: While working at Telecare ACT of Oregon, I have learned many things about the community I serve as well as the staff that dedicates their lives to service. Not only is my team, kind, thoughtful, compassionate and understanding, but they are also like family. Everyone very much works and lives by the Telecare core values and mission. On the ACT team each of these values are important to keep strong professional relationships and rapport with our members. Many of these values also touch upon the PSU values of diversity, critical thinking, communication and social responsibility.
